Tutorial - How to add Questionnaires

Step by step instructions on how to create questionnaires using lrnlab.

  1. Login to learnlab.
    • Navigate here and click on the unikey login link in the top right hand corner.
    • Login using your UniKey username and password.
    • You will then be asked to verify your information and register.
  2. Go to your folder.
    • Click on the my folder link in the top right hand corner of the page.
  3. Add a new sub folder inside your folder.
    • Click on the pull down menu and select Folder.
    • Click on the Add New Item button.
    • Fill in the form to add a new folder, make sure that you have filled in all of the required fields (items with a little red box next to them).
  4. Add a questionnaire to the sub folder.
    • You should now be in the contents view of the sub folder, select Questionnaire from the pull down menu and click on add new item.
  5. Edit the properties of the questionnaire
    • Click on the green properties tab.
    • Using this form you can add a description and title. You can use HTML in the description if you like as well.
    • If you want to allow users to edit the form without having to login you need to click on the "Allow Anonymous Access" checkbox.
    • Click on the change questionnaire button
  6. Add questions to the questionnaire
    • Click on the green edit tab
    • You can use this form to add new questions to the questionnaire.
    • You can ignore the Enabling Objective field
    • Let me know if you have troubles adding questions and I'll give you a more detailed description of how it works.
  7. View the questionnaire
    • At any time you can click on the green view tab to see the questionnaire as users see it.
  8. Add more questionnaires
    • You can add multiple questionnaires and if they're all in the same folder the submit button will forward to the next questionnaire.
    • To add questionnaires click on the green contents tab and repeat step 4.
  9. Publish the questionnaires
    • Click on the contents tab.
    • Click on the 'state: visible" pull down menu and select publish.
    • This will allow users that aren't logged in to view the contents of this folder.
  10. Review Results of the questionnaire
    • For each questionnaire click on the review results tab.
    • This page provides data in different formats, csv is suitable for importing into excel or SPSS.
